In an old, established restaurant, I saw a roach climbing the wall where a girlfriend and I sat. We had just ordered. I flagged down the waitress, told her what we saw and wanted the check to pay for the drinks she had just given us. She comped the drinks and I spoke to the manager on our way out. That image still freaks me out.

We went to an Hibachi restaurant in Philly. It's Japanese and they cook the food right at your table. The table is a large griddle surrounded by the wooden eating area. The tables seat 8, we went with 4 others, so they sat a young pregnant girl and her husband with us to complete the table. Half way through the dinner, the pregnant girl threw up, all over the griddle. Have you ever seen chunky throw up dancing on a hot griddle? Not very appetizing and the smell was horrific.

Our waiter! He was so rude! He practically threw the food at us, and obviously didn't want to work that day! Then half way through the meal, he left! So we had to get a new waiter!!

Sitting next to a table where some guy was gabbing away on his phone ! Loud and rude ! Also restaurants that are playing loud music when you want to have a nice peaceful dinner.

Revised answer: Last time I was at a restaurant, the people beside me and my friend were extremely messy. The restaurant provided complementary peanuts and a small bucket for the leftover shells. The patrons ate all of the peanuts and discarded the shells on the floor instead. I felt really bad for the waiter who had to clean it up.

Sapphire, actually some places encourage people to throw peanut shells on the floor. The oil that comes out of the shells when they are walked on and crushed protect hardwood floors. But if the restaurant provided a bucket for shells, you're right. They were just being messy. Also the restaurant should've had a sign on the front door alerting people with peanut allergies. :)

I was at a Diner in New York and ordered a piece of cheesecake for dessert. At first I thought there was a couple blueberries on it. After further inspection, it turned out to be mold. The server didn't even notice. YUKK!!! I'm glad I didn't automatically dig in.

My extended family went out for dinner, so there was close to a dozen of us. As the waitress was clearing the table she grabbed one too many plates, and was unfortunately directly behind me. The stack of dirty dishes went down along my back between me and the chair. If that wasn't bad enough she barely apologized and did little to help clean the mess. I have long hair and at the time it went nearly to my waist. I left with food and grease in my hair and on my clothes. You would think management would at least offer to pay for the cost of cleaning my outfit, but no. No discount on my meal either. I would have been happy if they offered a clean towel or two so I wouldn't transfer the mess to my car seat on the way home, and a sincere apology.

I thought bad service was a feature at Red Lobster. Had a gruelingly bad experience there (bad food, bad service, dirty tableware), and they paid for the meal and offered me a coupon for another meal. Like I would go back to that place. It has since closed.

Ordered fried eggs over easy.  They came over and really runny - barely cooked.  Tried to eat anyway as was in a hurry.  Second forkful came up with a really long black hair.  Breakfast was toast on the road.

Okay ... It didn't happen while *I* was in the restaurant, but ...

One of our local restaurants had an incident where a customer tried to set off a bomb.  He also had one at a nearby local tavern.  When neither went off as scheduled, the guy blew his brains out in the restaurant - during dinner rush hour - in the booth nearest the door, so everybody had to see it.

I've had black hair in my ice-cream, mines blonde. For me it's mostly the restaurants toilets, I'll eat the food then go to the toilets and think Jeez I hope the foods ok.
