To make cookie dough from scratch you will need to use the following ingredients: Butter, brown packed sugar, granulated sugar, eggs, vanilla, flour, baking soda and salt.

The amount that you use is dependent on the number of cookies that you require, with the proportions of the ingredients varying depending on individual tastes. To keep things as healthy as possible keep the sugar and salt input to a minimum; although eliminating them altogether would take away a lot of the taste.

By adding the ingredients into a large saucepan you will be able to blend them together. Get a good mix between the solid and liquid ingredients to blend the dough together, if you feel that there isn't enough of a particular ingredient don't be afraid to experiment by adding to the mix to get the dough the way that you like it.

Once the ingredients have been mixed up to the right levels you can cut the cookies to the required sizes. Place the cookies on a baking tray and place them in the oven at a medium temperature and in around ten minutes they will be firm and ready to be eaten.

Ingredients 3 cups all-purpose flou3/4 teaspoon baking powder

1 cup unsalted butter, softened
1 cup sugar
1 egg
1 tablespoon milk

Method
Place flour, baking powder, and salt in a work bowl, whisk together to combine. Set aside. Place butter and sugar in the bowl of a stand mixer and beat until light in color. Add egg and milk and beat to combine. Put mixer on low speed, gradually add flour, and beat until the dough comes together. Place
the dough on parchment paper and form into a log. Roll up in the wax
paper and refrigerate for at least 2 hours. This dough can be left in
the refrigerator for the next day, or frozen for a month.
Preheat oven to 375 degrees F.Now that the dough has harneded
in the refrigerator unwrap the parchment paper and slice your cookies
with a sharp knife to about 1/4 inch thick.Lay out your sliced dough on a cookie sheet and place back in the refrigerator for 10-20 minutes.Then bake for 10-14 minutes. Store in airtight container for up to 1 week.

A good answer, but there are so many different cookies, use a recipe.  You can get them off flour bags, out of books, or on the net.
